Testing the Utility of Collecting Blood Samples Electronically (TUBE)
Study Description
Wrong Blood in Tube (WBIT) errors are an important cause of ABO-mismatched transfusions. Recently, several institutions have implemented electronic patient identification (ID) systems designed to reduce patient ID errors at the time of phlebotomy. How effectively these systems improve patient safety is unclear. In this study, we will measure the impact of electronic patient ID by comparing WBIT rates on blood bank samples collected using electronic versus manual patient ID. Both concurrent and before-after analytic approaches will be used. We will also compare WBIT rates between institutions that use or do not use electronic patient ID (or alternative safety systems e.g. Typenex). Finally, we will review specific WBIT cases to characterize sources of residual risk to patients.
